When you come to the blood collection point, please bring the following essential documents with you:

• The request form signed by your doctor,
• An identity document,
• A ticket from your health insurance in your name.

The above documents must be provided upon arrival at the blood collection point. It is important to check the accuracy of your data marked on each sample is taken
